Community Nursing Service and You

All nurses carry photo identity badges.  If you do not recognise the nurse, please ask to see their identity badge before allowing them into your home.

We have a team of District Nurses attached to the practice who work geographically.

About the Community Nursing Team

Community Nurses are qualified nurses who have specialist understanding of caring for patients at home and in the community.

We aim to work with you and your family/carers to plan and agree an individual programme of care.

Where appropriate the Community Nursing team will help you to become as independent as possible.  This will involve teaching you and your carers how to manage your condition.

Student, (nursing and medical) may accompany the Community Nurse on occasion with your permission.

Home visits are provided according to genuine need.  Difficulty with transport to and from your surgery is not a valid reason to receive a home visit.

Confidentiality

Anything you discuss with members of the nursing team will be treated in confidence.

Information about you may be entered on to your computerised health record at your GP practice if appropriate.

Zero Tolerance

Verbal/physical abuse towards staff will not be tolerated. 
The Trust operates a Zero Tolerance Policy.
