How To Register

Our practice covers the following geographical areas Long Eaton, Breaston, Sawley, Toton and Attenborough. If you live in this area and would like to register with Aitune Medical Practice, please ask at our reception desk.

Please bring your medical card with you. You will be asked to complete a New Patient Form which will give us basic information until we receive your health record from your previous practice.

All new patients are invited to come for a Health Check with one of our practice nurses. When you come for your appointment please bring any medication that you are taking.

 Please let us know as soon as you change your name, address or telephone number.

A form is available at our reception for you to complete which should include all family members who are affected by this change.
