Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements.  This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

The NHS website has information on travel vaccinations with links to which vaccinations are needed for the country/ countries you are travelling to: NHS Travel Vaccinations

Travel health questionnaire

To help us offer the appropriate advice, please fill out the online form below before coming to see the nurse.

Travel assessment forms must be received at the surgery 3 months prior to travel.

Non-urgent advice: Please note

If your departure date is less than 2 months away, we cannot provide the vaccinations for you – you will need to attend a local travel clinic. The Care Navigators can provide this information for you if you are unable to find this for yourself

Travel Questionnaire

Please complete the form below to get more information about what travel immunisations you require. Most vaccines are given at least 2 weeks before travel, and some more complicated regimes take longer. Please try to give us prior notice (preferably 6 weeks).
