Non-urgent advice: Please note
All new patients are invited to come for a Health Check with one of our practice nurses. When you attend for your appointment please bring along any medication that you are taking.
Change of Personal Details
Please let us know as soon as possible if you change your name, address or telephone number.
A form is available at our reception desk for you to complete your new details, which should include all family members who are affected by this change.
Non-urgent advice: Please be aware
If a patient does not attend 3 or more appointments in a 6 month period, after an initial warning letter by the practice, that patient will be removed from the practice list if they continue to fail to attend.
